• Player's Last Name
• Home Address
• Subdivision or Complex
• Contact Phone Number
• Player's Date of Birth
• Player's Grade Level
• Player's School
• Player's Clothing Sizes (Jersey and Cap)

IMPORTANT NOTE ABOUT UNIFORMS:

      - Caps (Unused) can normally be exchanged without any issue.

      - Jerseys are not always easy to exchange.  Duplicate numbers are not permitted on any team so we may not have a replacement size on hand with an available number for your team.

      - Jerseys that have a sponsor printed on the back can NOT be exchanged.  A replacement would have to be purchased.

      - If you are not absolutely sure about the sizes you ordered, contact us as soon as possible.
